Since 1996, Avance Home Health has brought the healing home, with comprehensive services and quality care. Our number one goal is to assist patients with their recovery as soon as possible in their homes. To accomplish this goal, our nurses, therapists and social workers collaborate with referring physicians and families in developing the best course of home treatment.

Who is Eligible for Home Care?
To be eligible for home health services, a patient must:
- Have a medical condition that requires the skilled intermittent services of a registered professional.
- Be under the continuous care of a physician who is willing to work with a home health services organization.
- Be homebound. A patient is considered to be homebound if he/she has a condition due to illness or injury which restricts his/her ability to leave his/her place of residence except with the aid of supportive devices such as crutches, canes, wheelchairs, and walkers, the use of special transportation, or the assistance of another person or if leaving home is medically contraindicated.
